Up until the Covid 19 pandemic hit, Heritage Provider Network offered their patients' healthcare services by personally visiting them to do tests and provide them the attention they require. Elderly people being the primary service receiver of HPN, during the pandemic, they were unable to provide their services. Hence, HPN approached us with an exceptionally challenging and exciting product, A patient care management application. Which can store all the health parameters such as blood sugar, blood pressure, BMI and so on.

Our proposed solution is to build a patient care management system that could record the health status and details of patients in timely interval via just a mobile phone.
